Admission Documentation requests

Upon admission supporting medical records help us admit your loved ones and customize their care plan timely and efficiently.

  • A physicians assessment is often required to determine the medical necessity for long-term care. This will usually include a comprehensive review of the individual’s medical history, current health condition, and any ongoing treatments.

  • Information on the persons condition, such as chronic illness, disabilities, cognitive impairments, or mobility challenges.

  • A complete list of all medications the person is taking, including dosages and schedules.

  • Insurance information such as any Medicaid, Medicare, or private insurance plans. The facility needs to know any insurance information for billing purposes.

    Financial Information can be required for financial reviews to ensure an individual can pay for services. This may include income verification, assets, and other resources.
